Student Release Form Photo Release During the training course we will be taking photos for your assessment and for our own use. By signing below, you agree for us to use any photos of yourself, taken during the time you are at the Training Centre, including for: Assessment verification Advertising Training Our website Promotional material We will not use photos to slander or otherwise disadvantage participants or their respective businesses. If you do not wish for photos to be taken of yourself during the course, please let your trainer know.By signing below I acknowledge the above information. Signature Training Centre Induction For our commitment to safety, an induction tour of the Training Centre and the surrounding environment will be undertaken with everyone who participates in a training course at the premises. During the tour of the training centre, you will be shown the following: Location of emergency exits Location of fire extinguishers Location of first aid kits Location of emergency assembly area Restricted areas (that may only be entered with a staff member) Emergency shutdown button Rules & regulations of the training centre Bathroom facilities You will also be instructed on what to do in emergency situations and told the names of people who will be involved with the training course. By signing this form you acknowledge that you have been shown the above items, and been given instruction of what to do in an emergency situation. You also agree to abide by the rules and regulations of the training centre while you are on the premises. Signature
